Regence and Praxis Health Join Forces to Deliver Better Access to High-Value Care Across Oregon

Praxis Health and Regence BlueCross BlueShield of Oregon Launch Strategic Partnership to Improve Access to Advanced Primary Care Through New Clinics

In a major step toward reimagining the future of primary healthcare in Oregon, Praxis Health Advanced Primary Care and Regence BlueCross BlueShield of Oregon have announced a strategic collaboration to significantly expand access to high-quality, value-based care. This innovative partnership will officially begin delivering services on May 1, 2025, through the launch of two new clinics—one in West Portland and another in Eugene. These new facilities are specifically designed to serve Regence members and will focus on delivering comprehensive, patient-centered care in a modern, coordinated setting.

The joint initiative reflects both organizations’ commitment to addressing longstanding challenges in the healthcare system, including limited access to primary care, fragmented services, and inconsistent patient experiences. With an emphasis on whole-person care, these clinics will offer an integrated model that includes advanced primary care, chronic disease management, behavioral health support, and virtual care—all under one roof.

A New Vision for Accessible and Integrated Healthcare

At the heart of this partnership is a shared mission: to increase healthcare accessibility for Oregonians while ensuring that the care provided is personalized, proactive, and high-value. With many patients struggling to find timely access to primary care—especially for those managing complex or chronic conditions—Praxis and Regence are stepping in to offer a better solution.

The two new Praxis Health Advanced Primary Care clinics represent a reimagined model of healthcare delivery that breaks away from traditional, siloed approaches. Instead of treating physical and mental health as separate concerns or relegating chronic condition management to rushed follow-up appointments, these clinics will deploy a team-based strategy. Patients will have access to coordinated care teams consisting of primary care physicians, behavioral health professionals, care coordinators, and other clinical specialists who collaborate to support each patient’s individual health journey.

“This represents a bold commitment to redesigning the patient experience from the ground up—combining advanced primary care, next-generation technology, and a deeply human approach to access and care coordination,” said Dan McCarthy, CEO of Praxis Health. “We’re not just investing in better tools—we’re investing in a model of care that meets people where they are and delivers what they truly need.”

Designed for Regence Members, Built for Comprehensive Support

Unlike many traditional primary care models, which often take a one-size-fits-all approach, these new clinics have been purposefully designed to meet the specific needs of Regence BlueCross BlueShield of Oregon members. By tailoring services to this member base, Praxis and Regence aim to provide a seamless healthcare experience grounded in trust, familiarity, and long-term wellness.

Key services will include:

  • Preventive Care and Wellness Visits: Enabling members to stay ahead of potential health issues through regular screenings, immunizations, and personalized wellness planning.
  • Chronic Disease Management: Supporting individuals living with diabetes, hypertension, cardiovascular disease, and other long-term conditions with evidence-based treatment and care coordination.
  • Integrated Behavioral Health Services: Addressing mental health needs within the primary care setting to reduce stigma and provide faster, more convenient access to emotional and psychological support.
  • Care Navigation and Coordination: Helping members make informed decisions about their care journey, including referrals to specialists and support with social determinants of health.
  • Virtual Care Access: Providing flexible options for patients to connect with their care team from anywhere in Oregon, ensuring continuity of care even for those in rural or remote areas.

By integrating these services, the clinics create a “medical home” model that promotes holistic care and continuity, which is especially important for members who require regular monitoring or complex treatment plans.

Addressing a Critical Gap in Oregon’s Health System

For years, access to primary care across Oregon has been inconsistent, particularly in more densely populated urban areas like Portland and underserved regions such as parts of Eugene. Many residents have reported long wait times for appointments, difficulty finding providers accepting new patients, or a lack of integrated care coordination.

Recognizing this gap, Regence has taken proactive steps to ensure its members are no longer left navigating these challenges alone. “This novel collaboration with Praxis will address a need for access to high-value, high-quality care for Regence members—something we know has been an issue,” said Dr. Zak Ramadan-Jradi, head of Network Management and Strategy for Regence BlueCross BlueShield of Oregon. “Praxis has repeatedly stepped up to serve Regence members, and in this model, we know our members will benefit from being seen by a dedicated care team that will get to know our members, understand their concerns, and help keep them healthy—all close to home.”

Early Access and Enrollment Opportunities

To ensure a smooth rollout and to meet the anticipated demand, appointments at both clinics can already be booked. Regence members are encouraged to schedule early to secure timely access to wellness visits, preventive screenings, and consultations for ongoing health concerns. The clinics are currently accepting new patients and are staffed with experienced medical professionals ready to deliver compassionate, evidence-based care.

The opening of these clinics also reflects a long-term investment in community health and infrastructure. By locating the facilities in strategic areas of West Portland and Eugene, Praxis and Regence are making it easier for local residents to obtain regular care without the need to travel long distances or wait months for an available slot.

Technology-Enabled, Patient-Driven

A critical differentiator in this new care model is the use of technology to improve the patient experience. From streamlined scheduling and digital health records to remote consultations and real-time care coordination, the clinics are equipped with modern tools that enhance convenience without compromising the human element of care.

Patients will benefit from user-friendly online portals, secure messaging with care teams, and mobile access to health data—all features designed to empower individuals to take an active role in their own care. Meanwhile, physicians and staff will leverage data analytics and decision support tools to track outcomes, identify care gaps, and deliver personalized treatment plans.

A Scalable Model for the Future

The collaboration between Praxis Health and Regence BlueCross BlueShield of Oregon is more than just a two-clinic initiative—it is a blueprint for what the future of healthcare in the region could look like. If successful, this model could be expanded to additional locations, adapted for different populations, or used to influence policy around how health systems and payers work together.

In an era where the U.S. healthcare system is often criticized for its complexity and inefficiencies, partnerships like this offer a refreshing path forward. By centering care around patients’ real-world needs and leveraging the strengths of both providers and insurers, Praxis and Regence are aiming to deliver not just better health outcomes—but also a better experience for everyone involved.
