Superior HealthPlan Introduces Assessment Tool to Connect Texans with Community Support

Superior HealthPlan, a prominent managed care organization in Texas and part of Centene Corporation (NYSE: CNC), has launched a new screening tool designed to help Texans access community resources more easily. This assessment focuses on non-medical drivers of health—barriers that can affect healthcare access—including transportation, housing, food, and education.

Dr. Susan Mills, Senior Vice President of Population Health and Clinical Outcomes at Superior HealthPlan, stated, “Our clinical team will conduct this screening to identify a member’s immediate needs and help them find support quickly. After the assessment, we will follow up to ensure they received the necessary services and integrate these needs into their overall care plan.”

Launched on September 1, the screening tool is now part of a health risk assessment provided to members with complex care needs upon joining Superior HealthPlan and annually thereafter. Following the assessment, Superior employees will assist members in locating community resources through the online portal superiorhealthplan.findhelp.com, which offers access to free or low-cost services, including medical care, food, and job training.

Since January 2024, over 800 members have successfully accessed support through the FindHelp platform, which is on track to significantly increase the number of members receiving assistance compared to 2023. The new assessment is anticipated to further boost usage of this online resource.
