- Higi Stations: These health stations are placed in the community to screen for hypertension, obesity, and diabetes risks, promoting awareness and education.
- Resource Connection: The initiative connects residents with food insecurity resources, offering financial incentives through The Food Trust for community members and local store owners to stock fresh produce.
- Educational Materials: Health stations distribute information on heart disease risk factors and access to heart health resources via CardioSmart, ACC’s patient engagement program.
- Coalition of Partners: Collaborating with Modivcare, ACC, The Food Trust, and local store owners, the initiative aims to deliver essential health and nutrition resources to underserved communities.

- Risk Assessments: The Caring Hearts Higi Stations have conducted 400,000 heart risk tests nationally, providing valuable data on cardiovascular risk.
