Lightbeam Health Solutions, a leader in population health technology, Primary Partners, a Florida-based organization of over 750 independent physicians, and Alliance for Community Health (AFCH), a Florida nonprofit, have been honored with the 2024 SDOH Points of Light recognition by KLAS Research. This award acknowledges their effective collaboration in addressing patients’ social needs while delivering quality care. The 2024 SDOH Points of Light Report highlights exemplary partnerships that have made a significant impact on social determinants of health.
Lightbeam Health Solutions, in partnership with Primary Partners and the Alliance for Community Health (AFCH), has been recognized with two Points of Light case studies for their successful collaboration in addressing social determinants of health (SDOH). This partnership utilized Lightbeam’s advanced population health management platform to identify and manage patients with complex needs, particularly focusing on their underlying social needs. By leveraging this comprehensive platform, both independent healthcare providers and community-based organizations (CBOs) could analyze patient data, create personalized care plans, and track interventions at both the patient and population levels.
The initiative led to impressive results, including a 3% reduction in preventable readmissions, enhanced patient engagement, and a significant decrease in mortality rates among engaged patients, which dropped by half. In total, 301 patients with complex health and social needs showed marked improvement in their health outcomes due to the targeted outreach efforts.

The SDOH Points of Light recognition celebrates organizations that demonstrate effective collaboration, leading to shared outcomes in improving healthcare through the lens of social determinants. KLAS validated each submission through in-depth interviews with relevant stakeholders, gathering insights on engagement outcomes, best practices, and overall impact. Interviews for this recognition took place between May and July 2024.
The collaborative success of Lightbeam, Primary Partners, and AFCH was presented at the annual KLAS Social Determinants of Health Summit in Salt Lake City on October 8, 2024, in front of an exclusive audience of industry leaders, providers, payers, CBOs, and government representatives.
