Strive Health Expands Partnership with Zing Health for Value-Based Kidney Care

Strive Health, a national leader in value-based kidney care, has expanded its partnership with Zing Health, a prominent Medicare Advantage insurer, to provide specialized kidney care to all eligible members across Zing Health’s service areas.

Strive Health, a national leader in value-based kidney care, has announced the expansion of its partnership with Zing Health, a prominent Medicare Advantage insurer. This expanded collaboration will allow Strive to provide specialized kidney care to eligible Zing members across Ohio, Tennessee, and Mississippi, while continuing its coverage in Illinois, Indiana, and Michigan. With this growth, Strive is projected to serve thousands of Zing members with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) by 2025, anticipating significant double-digit growth this year.

Chronic kidney disease (CKD) affects millions of Americans, with over 130,000 new diagnoses annually. It accounts for 24% of Medicare spending each year and often leads to fragmented, high-cost care, which is primarily focused on managing symptoms rather than slowing disease progression. The expanded partnership between Strive Health and Zing Health aims to address this challenge by providing a more integrated, value-based approach to kidney care. By combining Zing’s tailored benefits with Strive’s proven value-based care model, members will have access to specialized nephrology care and coordinated care strategies that improve outcomes for the CKD population.

Chris Riopelle, Co-Founder and CEO of Strive Health, emphasized the importance of this collaboration: “Our partnership with Zing strengthens our shared commitment to delivering high-quality, coordinated care for individuals living with kidney disease. By integrating Strive’s value-based care model with Zing’s benefit plans, we proactively address the clinical challenges and social determinants of health that impact patient outcomes. This partnership creates a more unified and effective care experience.”

Zing Health’s Medicare Advantage plans are designed to meet the unique needs of individuals with chronic conditions, offering specialized benefits that address both medical and social needs. By working with Strive, Zing aims to provide a holistic approach to care that includes predictive analytics, interdisciplinary clinical teams, and high-touch patient engagement. This collaboration is intended to improve the quality of life for CKD patients while reducing avoidable hospitalizations and controlling the total cost of care.

Andrew Clifton, CEO of Zing Health, highlighted the significance of the partnership: “Our collaboration with Strive represents the future of Medicare Advantage, where members receive holistic, coordinated care from providers who understand their unique health needs. By integrating Zing’s benefits and member experience services with Strive’s high-quality clinical care, we’re ensuring our members don’t have to navigate the complex healthcare system alone. This partnership is a reflection of our commitment to improving outcomes for those managing kidney disease.”

The expanded partnership will integrate Strive’s unique value-based care model with Zing’s benefits and services to deliver a seamless care experience for members. By leveraging technology-driven care interventions, predictive analytics, and the support of local providers, Strive is creating an integrated care delivery system that supports patients throughout their kidney disease journey—from the early stages of CKD to ESKD. This model emphasizes preventive care, early intervention, and ongoing management to help reduce the need for costly and invasive treatments later in the disease progression.

Strive Health partners with commercial and Medicare Advantage payors, Medicare, health systems, and physicians, offering flexible value-based payment arrangements, including risk-based programs. With its technology platform, CareMultiplier™, Strive enables the delivery of personalized, data-driven care to over 121,000 people with CKD and ESKD across all 50 states. Additionally, Strive partners with over 6,500 providers to ensure coordinated care that supports the needs of patients at every stage of their disease.

Strive Health’s commitment to quality care is reflected in its NCQA-accredited case management and population health programs. Its technology platform, CareMultiplier™, is also HITRUST certified, ensuring that patient data is protected and used effectively to improve outcomes. By expanding its partnership with Zing Health, Strive continues to push the boundaries of kidney care, aiming to transform the healthcare experience for people living with kidney disease through value-based, patient-centered approaches.

This expanded collaboration between Strive Health and Zing Health marks a significant step forward in improving kidney care, providing patients with better access to the care and resources they need to manage their conditions effectively. Through this partnership, Strive and Zing are poised to make a meaningful impact on the lives of those with CKD and ESKD while advancing the value-based care model within the Medicare Advantage system.
