Hackensack Meridian Health is launching HMH 24/7, an AI-driven platform designed to connect patients with primary care services through a blend of virtual and in-person care. This new solution, developed in collaboration with K Health, offers a seamless integration with Hackensack Meridian Health’s existing ecosystem, ensuring continuity of care and a personalized patient experience.
HMH 24/7 provides patients with easy access to clinicians for various primary care needs—sick, chronic, and preventive care—directly from their phones. Each patient is paired with a dedicated clinician, and all interactions are recorded in their Hackensack Meridian Health medical record, creating a connected care experience. K Health, known for its work with leading health systems like Cedars-Sinai, is instrumental in developing this innovative solution.
