HealthEC’s technology will integrate Electronic Health Records and workflows, ensuring seamless continuity of care and coordinated service planning for individuals requiring crisis stabilization and connection to community resources post-behavioral health hospitalization. By linking patient care plans with Collaborative Bridges, HealthEC enables care coordinators to promptly follow up with patients after discharge, a crucial step given the heightened risk of suicide during the first week post-discharge.
